DE GUIA 03:16 PM, Aug 16, 2023

To whom it may concern,

I would like to change my birthdate on my vaccination card and VAXCertPH record and also add a suffix to my name. I am not sure who put my birthdate incorrectly. Please change it from AUGUST 03, 2003 to SEPTEMBER 06, 1998 because I'm afraid that I might get into problems in the future with government documents.
