Good day! 

This is Baby Joy Morales, 31 years old. I was born in Sto.Niño, Kiblawan, Davao Del Sur. I would like to request for Baranggay Immunization records because unfortunately my mother loss my immunization/vaccination card. This information will be of use for USA immigration purposes. I am looking forward to an immediate and a positive response.

Thank you for your request dated Apr 25, 2023 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your request

You asked for Immunization records from 1992 to 2023.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. 

We regret to inform you that DOH does not have custody of the barangay immunization records and this is handled by the barangay where your mother received her immunization. Also, we cannot disclose immunization records in the eFOI portal as it contains sensitive personal information covered under the Data Privacy Act. 

We ask that you directly coordinate with the concerned health center for this request.

Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
