Visit the DOH National Health Facility Registry (NHFR) website at https://nhfr.doh.gov.ph/VActivefacilitiesList for the list of the Health facilities in the Philippines. You can select the health facility type and download the data in Excel or CSV format

Please note that we are only able to provide some of the information you have requested for as we do not collect data on facilities that offer diabetic services. We ask that you directly coordinate with the facilities indicated at the NHFR website to support your study
