I would like to request a copy of the death certificate of Bernardo Tarcela Austero, my grandfather.
He died at the Philippine General Hospital last April 27, 1961. 
I do hope for your kind consideration regarding this matter.

While our aim is to provide information whenever possible, in this instance this Office does not have [some of]* the information you have requested. However, you may wish to contact Philippine General Hospital (PGH) as this hospital is not under the jurisdiction of the Department of Health.
