i would like to request information regarding on my swab test result, the swab test dated last january 7 2022 at San Juan Gym San juan city
I cant track my result because i forgot my barcode

You asked for Barcode  number.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. Please be informed that test results may not be requested from and released through the FOI portal as this constitutes a violation of the Data Privacy Act. Release and dissemination of the requested results is prohibited because it contains personal information of patients not for public consumption.

For follow ups regarding the results of swab tests and your request for barcode number, the testing center which conducted the swabbing would be the appropriate office to address this request.

Your right to request a review
