Good day! I would just like to know about the health status of the people in the community of Tarlac but I'm not sure about the specific title of the data to request. Would it be possible to request for information if there is a 
-Tarlac Community Health Survey for Mental Health and Well-being
- Summary of Medical Report indicating the number of patients from Tarlac City (if possible) or;
-Any related documents about the health status of Tarlac City
