Good Day. I want to raise a concern regarding my lost vaccination card. Is there a way to ask for a copy of it. I was vaccinated at Citi Mall, Iponan, Cagayan de Oro. My Ist dose was October 13, 2021 and 2nd dose, November 13 2021. I also got a booster shot on June 9, 2022 happened at Centrio Ayala Mall. These are correct details. I want to ask for a copy of it for important references or identity cards that may be needed later on. Thankyou in advance.

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
