I would like to request to update my vaxcert record. 
It only shows 1st dose and no 2nd dose on my vaxcert.

Birthdate: November 29, 1989 ( I have a pending req ticket as well on this to change my birthdate from 11/29/1989 to 11/11/1989 ) can also refer to this ticket number #DOH-847463663316
1st Dose : 11/11/21 MODERNA in Dipolog City

Please prepare your reference number generated thru Vaxcert.doh.gov.ph for verification purposes and ease of transaction.
