Good Day FOI,

I would like to ask request on DOH to update my vaccine record since my 2ND vaccine shot does not yet appear. I will be using the details to work abroad.

Your FOI request is approved.

Please be informed that the encoding of
vaccination records is done by the concerned
vaccination center. If your vaccination record is
unavailable, the record may not have been
uploaded yet in the Vaccination Information
Management System (VIMS) by the vaccination
center.
For further assistance, you may contact the Vax Cert
Customer service. Here are the contact

Kindly coordinate also with the LGU where you
received your vaccination and request for
assistance to generate your vaccination
certificate.
