Hi! I would like to change the lot number for my SECOND Dose of Vaccination hence it appears in my VaxCertPh that my first and second dose have the same lot number. The lot number for my second dose should be 32025BD not FF8847. Can you process it right away for this will be use for abroad purposes and my flight is fast approaching. Thank you so much.

You asked for Change of Lot Number in Vaccination Certificate.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. 

Please be advised that requests for correction in the vaccination certificate is done by the LGU where you received your vaccination. We request you directly coordinate with the LGU on your request for change of Lot Number in Vaccination Certificate. Kindly present your copy of vaccination card for validation of the request
