Your request You asked for Change of Lot Number in Vaccination Certificate. Response to your request While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. Please be advised that requests for correction in the vaccination certificate is done by the LGU where you received your vaccination. We request you directly coordinate with the LGU on your request for change of Lot Number in Vaccination Certificate.
