I would like to raise a concern regarding my vaccination record. I lost my vaccination card in year 2022 at SM Molino. I'm fully vaccinated but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they only recorded my first dose and my second dose is not recorded.

I was vaccinated in SM Bacoor in their free vaccine program called Ligtas Bacoor #BAcoorYesSaBAkuna in the city of Bacoor, Cavite . My first dose was on November 11, 2021 and my second dose was on December 2, 2021.  I have a pic of my vaccination before it was lost. Waiting for your feedback, thank you so much

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
