Dear FOI,

I  would like to have my son Alpriam Sevric Lising Dionisio's Vaxcert updated in the system as his first dose is not recorded. I will be needing the vaxcert to travel local with him. I already emailed vaxcert support but not replying to me for a month now.

Below is his vaccination details:

First dose (Pfizer ) - 02/08/2022 Manila City (NO RECORD)
Second dose (Pfizer ) - 03/01/2022 Manila City

Kindly coordinate directly with the National Patient Navigation and Referral Center (NPNRC) at Telephone number (02)8651-7800 local 2431-2345 or hotline number 1555 local 2 for immediate assistance.

Please prepare your reference number generated thru Vaxcert.doh.gov.ph for verification purposes and ease of transaction.
