Good evening! I just like to raise a concern regarding my vaccination card. I lost my wallet where my vaccination card where around September or October of 2023. I'm fully vaccinated but I don't have other copy of my vaccination card. I was vaccinated in Karmelli Medical Hospital, Laoag City, Ilocos Norte. My first dose was on October 02, 2021 and the second dose was at December 11, 2021. As I have said above I lost my vaccination card but fortunately, I was able to take a photo of it. In this case, I would like to know what I can do about this matter? Thank you.

About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record before the issuance of a new vaccination card.
For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
