Good day!
I would like to raise a concern regarding my vaccination record. I lost my vaccination card last year. I'm fully vaccinated but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my vaccination. 

I was vaccinated in Pililla, Rizal. My first dose was on November 15, 2021 and the second dose was at February 7, 2022 while my booster shot was given on March 4, 2023. As I mentioned above, I lost my vaccination card and wasn't able to take photo of it.

You asked for Lost Vaccination Card - not found online record of vaccination.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
