Hello, my name is Julie Ann Abkilan-Gipanago. I'm requesting a Certificate of Employment under the Department of Health - Western Visayas Region. I was assigned to Bacolod City From July 2020 to February 2022 as a Disease Surveillance Officer and Vaccinator.

Would it be possible to have the Certificate of Employment under my maiden name - Julie Ann Abkilan. Thank you!

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have  the information you have requested. 

We regret to inform you that it is not possible to have your Cert of Employment under your maiden name,  Julie Ann Abkilan, for the reason that the documents and the contract that you signed was under your married name.

Attached is the letter from DOH regional Office 6 for your reference
