I would like to raise a concern regarding my vaccination record. I lost my vaccination card last April 2023. I'm fully vaccinated but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my vaccination. 

I was vaccinated in Biñan City, Laguna in Alonte Sports Arena. My first dose was on November 18, 2021 and the second dose was at December 9, 2021. As I have said above I lost my vaccination card and wasn't able to take photo of it, but I do have pic of proof.

with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
