Good Day FOI,

I would like to request on DOH to update my vaccine record since my 2nd booster shot does not appear yet despite my request last December 7, 2022. For reference below are the reference IDs:
City of Taguig: 1805060
City of Manila: 1805061

Here's the summary of my vaccination details:

1st dose: AstraZeneca 07/29/2021 City of Taguig
2nd dose: AstraZeneca 09/23/2021 City of Taguig
1st Booster shot: Pfizer 02/04/2022 City of Taguig
2nd Booster shot: Pfizer 12/01/2022 City of Manila

You asked for Vaccination Correction Request.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.
