I would like to change my birthdate on my vaccine card. I've written the wrong birthdate when I was signing up for my 1st dose. Please change it from December 23,1999 to December 02,1999. I might run into some problems in the future with any of my government documents.

You asked for Wrong Birthdate of my vaccination card.
Response to Request
Your FOI request is approved.
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate.
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
