Hi, I would like to inquire if I am still eligible for OCA / HEA from a previous company I worked with.

PREVIOUS EMPLOYER: Healthcube Medical Clinics
Address: 241 Shaw Blvd, Mandaluyong City
Duration: March 2017 to Aug 2021
Status: Resignation

I have not received any call from my previous employer since I resigned.  I would like to inquire if my OCA/HEA was remitted with them during those times.  Hope to receive feedback from you soon.  Thank you.
