Thank you for your request dated Feb 25, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Immunization Records.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. 

We wish to inform you that we do not have custody of immunization records. We suggest that you coordinate with the health center of Manggahan, Gao, Quezon City or through the Quezon City Health Department of the QC LGU for this request
