I would like to raise a concern regarding my vaccination record. I lost my vaccination card this February. I'm fully vaccinated but I don't have other copy of my vaccination card. I would like to request another vaccine card and Vaccine Certificate because mine got lost. I dont have any pictures of my vaccine card however I remember when and where I got vaccinated. I got my first dose at December 1, 2021 in Ibajay Aklan at ABL Sports Complex Bakuna Center and my second dose at January 3, 2022 at the same place. I tried searching for my record in VaxCertPh, but no record.

with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
