You asked for Complete list of Mortality - includes depression and anxiety.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have [some of]* the information you have requested. The National Mental Health Program currently does not have ANY OFFICIAL data on the statistics on mental health disorders in our country in all its aspects, including depression and anxiety. The program is currently developing a registry and information system to gather these information. We hope they can await as the department, more specifically, the program strives to address their concerns and strengthen mental health in our country. 

We are only able to provide the list of Accredited Psychiatric Facilities in the Philippines. For further reference, please visit the link below:

 We are can also provide you the regional level data on Alzheimer's Disease Mortality Rate for 2016. You may view this in the Philippine Health Statistics Annual Report at https://www.doh.gov.ph/sites/default/files/publications/2016_PHILIPPINE-HEALTH-STATISTICS.pdf
