I am Mary Irene Nobleza, a registered nurse. I am writing to inform your good office that my request for an updated digital vaccination certification is still pending for 19 days now. I've requested for it as a prerequisite for my UK deployment this month. My reference number is 4404026 (Iloilo City). I hope you can assist me on this matter as soon as possible.

You asked for VAXCERT REQUEST PENDING.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
