Good day! I would like to request the following data for my group's ongoing research regarding Antimicrobial Stewardship practices of Community Pharmacists in Davao City, pre- and post-pandemic.
1. Total number and list of all community pharmacies in Davao City as of 2023

d) contacts or socials (phone number and email address)
2. List of all actively practicing community pharmacists in Davao City from pre-pandemic until post-pandemic/today (at least from 2018 to 2023)

You asked for Community Pharmacies and Pharmacists of Davao City as of 2023.
Response to Request
Your FOI request is approved.
The information requested may be accessed via FDA Verification Portal (https://verification.fda.gov.ph/Home.php) and specific information (e.g., license number, name of establishment, owner, address, activity, issuance date, and expiry date) can be obtained.
Step 1. On the FDA Verification Portal, click the “Drugstore” under “Drug Industry” Tab
Step 2. Click the “Search” Drop-Down Menu and click “Exact match”
Step 3. On the search field, type the details/information needed. You may search the name of every City and Municipality in NCR.
Step 4. Furthermore, you can also download or export the list into an excel file for ease of navigation or filtering of names of chain pharmacies/drugstore.
Kindly take note that the FDA issues a License to Operate (LTO) to Drugstores provided that a pharmacist is employed as the qualified personnel. The number of pharmacists depends on the operating hours of the store only.
