1. Requesting a medical assistance for my medicines that i had to buy and take everyday

In addition, kindly submit the following documents to the Malasakit Program Office or the Malasakit Centers

1. Medical Certificate or medical abstract
2. Certificate of Indigency
3. Drug or Medicine prescription or treatment protocol, as applicable
4. Laboratory or diagnostic request
5. Hospital bill
