While our aim is to provide information whenever possible, in this instance this Office does not have  the information you have requested. 

We regret to inform you that the Philippine Center for Disease Control is still in development and for finalization, hence this is not yet available, 

For now, may we refer you to the Health Facilities and Services Regulatory Bureau (HFSRB) website at hfsrb.doh.gov.ph/licensing-accreditation-of-health-facilities-and-services/ for the licensing and accreditation of health facilities.
