Your request You asked for 2009 Nationwide Baseline Summary Consumer Participation. Response to your request While our aim is to provide information whenever possible, in this instance this Office does not have [some of]* the information you have requested. As per information from the Bureau of Local Health Systems Development, we regret to inform you that the requested document is no longer available in the records of BLHSD nor the Department Library The said baseline summary is a one time survey conducted as one of the requirements for the Asian Development Bank grants for FY 2009.
