Good Day,

I would like to raise a concern regarding my vaccination record. I lost my vaccination card this July. I'm fully vaccinated but I don't have any copy of my vaccination card. I was vaccinated in City of Mandaluyong, NCR (Mandavax). My first dose was on February 1, 2022 and the second dose was at May 23, 2022. 

I would like to request a new vaccination card for medical purposes if it is allowed.

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
