3. Vaccination Status of dialysis patients.

Your FOI request is approved. 

Attached are available data on Chronic Kidney Diseases. Visit the Health Facilities and Services Regulatory Bureau (HFSRB) at the link: https://hfsrb.doh.gov.ph/list-of-licensed-health-facilities/ for the DOH accredited health facilities

Please note that we are only able to provide some of the information you have requested for as we do not have Vaccination Status of dialysis patients.
