I lost my vaccination card but I do have a photocopy of it. How can I request for a new official one? I will be needing this for school purposes. How long can it take to get another copy?

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
