I would like to raise a concern regarding my vaccination record. I lost my vaccination card this month and I'm fully vaccinated.

I was vaccinated in Alonte sports arena, City of Biñan , Laguna.  As I have said above I lost my vaccination card and wasn't able to take photo of it and I don't remember when did I got my first and 2nd dose. I hope you will help me Thank you.

Response to Request

Your FOI request is approved. 
About the copy of vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
