Hi! I would like to request to correct the surname in my Covid-19 Vaccination record for me to be able to print my VaxCert for my upcoming flight 1 week from now. The name listed is JHOANA BAYOT NONO and recently my Birthcertificate was updated to JHOANA BAYOT. The (NONO) in the surname is no longer exist. will be needing the correct information for my vaxcert asap for working abroad purposes.

Your FOI request is approved.

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.

If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
