 I want to request the record of my measles, dengue, and polio immunization and any immunization that I've taken from the date Jan. 1, 1978, to Jan. 1, 1984.
My name is Rostum Allion Tubelleza, born on Feb.11, 1971, in Manila, Phils.

You asked for measles and dengue.

Response to your request

While our aim is to provide information whenever possible, we regret to inform you that DOH does not have custody of immunization records. We ask that you directly coordinate with the health center where you  received your vaccination or you may coordinate with the Manila Health Department of Manila LGU for further assistance.

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to the Central Appeals and Review Committee, at foi@doh.gov.ph.
