Hi, Good Day. I would like to request if you can get a record of my vaccination abroad. I lost my vaccination card(booster). I was boosted with Pfizer while working onboard a cruise ship(Navigator of the seas-Royal carribean cruise line) that was in Los Angeles, California. Below are my infos. Please if you can help me. Thank you in advance.

You asked for Vaccination record. 

Response to your request

Since you were not vaccinated in the Philippines, Unfortunately, the vaccination records may not be accessible by the central database which holds the data of recipients of the COVID-19 vaccine from the Philippines. Kindly request for proof of vaccination (i.e. vaccination card/vaccination certificate) from the country where you received your primary dose series, provided it has all the necessary information (personal details and vaccination details)
