Hi, I would like to request the following:

 - The data of medicines disbursed to patients/residents of health units in Pampanga e.g. name of medicines, dosage, quantity
 - The demographics of residents who acquired medicines among health units such as sex, age, illness, etc. 
 
Kindly remove any exclude data that will identify a specific person, pursuant to RA 10173.

The data will be used for developing an application/ software using data analytics and machine learning.

Your request is approved. We regret to inform you that the Central Luzon Center for Health Development (DOH Regional Office 3 in Pampanga) are unable to provide the requested data because these date are not being captured by their routine reports. 

Your right to request a review

David, Chair of Data Governance Oversight Committee/ DOH Central Appeals and Review Committee, at foi@doh.gov.ph.
