Hi! I am Mark Daryl Calzita Calites, was born on February 6, 2002 and I would like to ask for a vaccination certificate or information as a proof of my vaccination since I lost my Vaccination card and it is required on my new job. It is totally nowhere to be found.

I got vaccinated year 2021 in the city of Mabalacat, Province of Pampanga

with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
