I would like to raise a concern regarding my vaccination record. I lost my vaccination card this july. I'm fully vaccinated but I don't have other copy of my vaccination card. I was vaccinated in brgy. Sarmiento, Parang, Maguindanao. My first dose was on December 23, 2021 and the second dose was at December 28, 2021. As I have said above I lost my vaccination card and wasn't able to take photo of it.

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
