Hi, Good day to you!
I would like to address a concern regarding my vaccination record. Unfortunately, I lost my vaccination card in late 2022. I have been fully vaccinated, but regrettably, I do not possess another copy of my vaccination card. My vaccinations were administered in Carmen, Cebu, under the LGU. The first dose was given on December 27, 2021, and the second dose on January 7, 2022. I lost my vaccination card and was unable to take a photo of it as my old phone was also lost along with the card.

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
