I would like to raise a concern regarding my vaccination record. I lost my vaccination card here in korea I'm fully vaccinated.i will having a vacation to Philippines tomorrow 22 until 30th is my flight back here in Korea. I need to know some info of my vacation so i can recover it.incase they need  at the airport my vacation card or vaccine certificate i will be able to present it.

Im julie sevilla jacobe. As i remember i was vaccinated in montalban rizal.My first and second dose was on January 2022. As I have said above I lost my vaccination card and wasn't able to take photo of it.

You asked for I lost my vacation card here in korea.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.
