I refer to my letter of Dec 28, 2020 07:42:04 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Annual Health Facility Statistical Report.

In order for us to work further on your request, we need more information from you regarding your query.According to the DOH Regional Office IV-A, they will not be able to provide the whole report of the Annual Health Facility Statistical Report of Taal Polymedic Hospital and Medical Center. We would like to ask you what specific data that you need?

Hello. I want to ask for discharge data (table), ten leading causes of morbidity/mortality disaggregated to age and sex (table), and summary of patients in the hospital (plus bed occupancy rate). Thank you.

You asked for Annual Health Facility Statistical Report.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have [some of]* the information you have requested. This is in reference to a request for AHFSR of Taal Polymedic Hospital and Medical Center for 2019. Unfortunately, the data for the said request is not yet available as advised by our DOH Regional Office 4A - Regional Licensing and Enforcement Division (RLED).
