Your Request

You asked for REQUEST OF INFORMATION AND ENDORSEMENT CERTIFICATE.

Response to Request

As per Health Facility Development Bureau (HFDB), please be informed that a Certificate of Appearance can only be requested by clients who have physically appeared in our office. Based on their Customer Feedback Survey for walk-in clients, you have not physically availed of technical assistance from their office.

We regret to inform you that HFDB do not issue an endorsement certificate indicating their approval/support to proposed architectural thesis. HFDB believe that they are in no position to approve/endorse your thesis proposal since this is an academic undertaking and such approval and assessment should be in the purview of your institution.

In lieu of their approval/endorsement, they recommend alignment of your proposals to the Philippine Health Facility Development Plan (PHFDP) 2020-2040, as the macro plan for the development of priority health facilities in the country towards the realization of the provisions in the Universal Health Care Act. The direction of the Department of Health is to establish Specialty Centers by designating existing DOH hospitals across the country to be capacitated in specialty care. This strategy would enable the government to maximize its limited resources while ensuring the provision of specialty care services to all.

As per Health Facility Development Bureau (HFDB), please be informed that The Department of Health has developmental standards for general hospitals and selected health care facilities only. Administrative Order No. 2012-0012 Rules and Regulations Governing New Classification of Hospitals & Other Health Facilities articulates the different classifications and service capabilities of health care facilities. Should you find the facility you have in mind under classifications in the above mentioned document, you can refer to the Administrative Order No. 2016-0042 Guidelines in the Application of DOH Permit to Construct for the planning and design guidelines of specific health care facilities. You may access the mentioned policies at the official website of the Health Facilities and Services Regulatory Bureau of the DOH.
