Good Day FOI,

I would like to ask request on DOH to update my vaccine record since my booster shot does not yet appear. I will be using the details to travel abroad. 

Here are my details:
Origin: Iloilo

1st dose: Sinovac 8/31/2021 Badiangan, Iloilo
2nd dose: Sinovac 9/28/2021 Badiangan, Iloilo
Booster shot: Astrazeneca 1/11/2022 Janiuay, Iloilo
