Greetings!

Thank you for your request dated Aug 24, 2023 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for baranggay immunization records.

Response to your request

While our aim is to provide information whenever possible, in this instance, we regret to inform you that we do not have custody of immunization record. We ask that you coordinate with the Barangay Health Center/Rural Health Unit  where you received the immunization for this request
