I would like to raise a concern regarding my vaccination record. I lost my vaccination card last year.  I can't remember the dates of my vaccination my name is Vina A. Galicia and my vaccine is Pfizer

Please help me

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
