Good afternoon ..i was vaccinated last year in Davao city. Unfortunately, my vaccination card burned because of fire incident last February 25.Can i have a booster certificate?  Thanks a lot

About the lost/burnt vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
