Hi I would like to seek replacement for my lost vaccination card. Under the name of Joseph Bisnar Consular Jr. Birthdate: September 19,1997. Vaccinated year 2021 with Pfizer at SM Seaside City Cebu, fully vaccinated but not with boosters. Hope you can help me.

with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
