While our aim is to provide information whenever possible, in this instance this Office does not have  the information you have requested. 

We regret to inform you that FDA does not gather this information and this data can only be gathered from the manufacturer and importers.

We ask that you visit the FDA website at https://www.fda.gov.ph/ and the FDA Verification portal at https://verification.fda.gov.ph/Home.php for the comprehensive list of Establishments and Health Products with License to Operate (LTO) and Certificate of Product Registration/Notification (CPR/NN) respectively.
