Good day na admit po ang wife ko December 9,2023 naoperahan sya sa Galstone and naging ok within 3days and doctors already said pwede na sya makalabas nun mag ask kami sa billing office sabi need daw bayaran yun buong bill bago kami makalabas kaso wala po kami pambayad na deduct npo yun sa HMO and Philhealth nya ang natira po is almost 90k out of 172k na bill nmin for now room na lng ang binabayaran nmin na dumadagdag pa sa billi and kahit po mag iwan daw ako ng laptop ko eh ayaw nila kailangan daw may pumasok from PCSO kaso as of today may problema sa system nila

Please make a formal letter addressed to Engr. Annette B. Tan, RN, MPH, PSMFPM, CESE, OIC-Director IV of the Health Facilities and Services Regulatory Bureau (HFSRB), including the following:

1. Name of the health facility (hospital) and its address.

2. Name of the patient.

3. A discharge order from the attending physician or doctor.

4. State the specific request for assistance.

and send an email to hfsrb@doh.gov.ph

Your right to request a review

If you are unhappy with this response to your FOI
request, you may ask us to carry out an internal

Appeals and Review Committee, at
foi@doh.gov.ph. Your review request should
