Hi! The baby book of my child containing her immunization record since birth is missing. She is vaccinated in Mapulang Lupa Valenzuela City and RHU Nabua, Camarines Sur And now, the school that she is about to enter is asking for it. Could I request for a duplicate copy of her immunization record? Thank you

We regret to inform you that we do not have custody of the  immunization record. We ask that you coordinate directly with the health center or facility where you received your child's immunization record for this request.
