Hello, I would like to request my official vaccine certificate that would reflect my my booster shot. It is a requirement for my travel to Manila and a requirement for a job in Singapore. Thank you!

You asked for Vaxcertph with update booster.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

Rest-assured that we've already coordinated with Agent 201 from the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. Please expect a call or an e-mail from them, if none, kindly call them back through the number above since they informed us that you haven't replied to their email for clarification last May 2023.

In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
